What are Forms?

Forms are helpful tools for events, volunteer projects, or volunteer events, where admins can capture acknowledgements of a waiver prior to participating. Build custom forms for your colleagues to fill out directly in the Deed app and collect all the info your organization requires, seamlessly.

Example Forms may include:

  • COVID-19 Health Attestation
  • Photo Release Form
  • Mailing address to send Swag
  • Event suggestions

Create a Form

On the Admin hub, click on the Forms tab and select "Create a new form" in the upper right hand corner. From there, you can add questions and upload any relevant documents like waivers. Once created, your form will be stored on the “Forms” tab for future use and editing. You can use any of the popular sections that are available or add as many custom questions as you need.

❗ If you are attaching a pre-created waiver, you should edit the document to include any information that needs that needs to be consented to and remove check boxes or places for a signature. You can then create a simple “Checkbox” question for employees to click and consent.

Pro-tip: A Form cannot be edited after a response is recorded. Please double check that you've included all relevant questions or documents in the form before you tag it to a volunteer event.

 

Add Forms to Deeds

When creating an Event, Volunteer Event or Volunteer project under the “Manage Deeds” tab, you can add the Form(s) you created under the step "Volunteer Forms." All employee form responses are stored on the volunteer’s profile in the directory.

Pro-tip: You should create your form before you create Deed to make things easier. However if you’ve already created your Deed, you can edit those details and add your form. Learn more here!

 

Export Responses

To export your Form responses, click on the deed you've added in 'Manage Deeds.' Then, select 'Export Responses' to download a .csv file with all of the relevant information.

Important: Form responses are recorded in relation to the Deed they are attached to. For example, if you tag the same Form to several deeds, you can only export responses from each Deed individually.
