Forms differ from Surveys in that they allow you to collect information about your employees as they are signing up for a specific deed.

Audience	Participants in a specific deed.	Participants in a specific deed and/or anyone you send the link to.
Relation to Deeds	Must be related to a specific deed and added during the deed creation process	Stand-alone, can be unrelated to a specific deed.
